Liability, Health, & Medical Acknowledgment

At Culinary Concepts AB, we take every precaution to create a safe, supervised, and professional environment. However, a kitchen is a dynamic space involving heat, sharp tools, and active movement. By joining us in the “Lab,” we ask that you acknowledge the inherent risks and provide us with the authority to act quickly in your child’s best interest.
1. Assumption of Risk I understand that participation in a culinary program involves certain risks, including but not limited to cuts, burns, and slips. I voluntarily assume these risks on behalf of my Junior Creator and release Culinary Concepts AB, its staff, and its partners from any liability for personal injury or property damage that may occur during the program.
2. Emergency Medical Authorization In the event of a medical emergency, the CCAB team will first attempt to contact the primary guardian listed on this form. If we cannot reach you, I hereby authorize the Lead Chef or on-site staff to:

Administer basic first aid.

Seek emergency medical treatment (including transportation to a hospital).

Consent to any necessary medical procedures as recommended by a licensed physician.

Note: Any costs associated with emergency medical care are the responsibility of the parent/guardian.
3. Health & Medications

The Essentials: If my child requires a life-saving device (EpiPen, Inhaler, etc.), I agree to provide it to the Lead Chef on the first day of camp and ensure it is clearly labeled.

Daily Health: To keep our kitchen healthy, I agree to keep my Junior Creator home if they are showing any signs of contagious illness.
4. Allergic Awareness & Ingredients I have provided a full list of my child’s food allergies and sensitivities in the registration profile. I acknowledge that while CCAB is a professional facility that practices cross-contamination prevention, we are not a certified “allergen-free” kitchen (e.g., we may use flour, nuts, or dairy in other recipes).
